
Why Does My Body Still Feel Like It’s at Work?
There is a particular kind of exhaustion that happens after a nursing shift when you are technically home, yet some part of you is still at the hospital. Dinner is happening, someone is asking about your day, and your brain is quietly reviewing whether you charted that one thing, wondering how a patient is doing, or thinking ahead to tomorrow’s staffing problem.
Your shift ended. Your nervous system may not have gotten the message yet.
We tend to interpret this as an inability to relax or a failure to “leave work at work,” but I think that explanation misses something important about how the nervous system actually functions. Nurses spend hours adapting to an environment that requires constant attention, rapid decision-making, pattern recognition, and the ability to respond when something changes. It makes sense that the body does not immediately abandon those patterns simply because you walked through a different door.
Your Nervous System Has Been Learning All Day
Healthcare may be unpredictable, but experienced nurses become extraordinarily good at finding patterns inside that unpredictability. You recognize the sound of an IV pump without looking up. You notice a subtle change in someone’s breathing before the monitor gives you a number to explain it. You can walk into a room and know something is different even when you cannot immediately articulate what caught your attention.
Yes this is nurse intuition AND also that your brain has spent years collecting information, comparing current circumstances with previous experiences, and learning which details matter.
The nervous system is constantly making predictions. It uses what has happened before to help determine what might happen next and how much attention or physiological preparation the current situation requires. That ability is enormously useful in healthcare, where recognizing a change quickly can matter.
The same learning does not disappear at the end of the shift.
If your nervous system has spent twelve hours listening, scanning, prioritizing, anticipating, and responding, it has been practicing a very specific way of interacting with the environment. Clocking out changes your schedule. It does not necessarily produce an immediate physiological change.
The Commute Is More Than Dead Space
I used to think of the drive home as the space between two parts of my day. Work was finished, home had not quite started, and the commute was simply how I got from one to the other.
Now I recognize that space is a transition.
You can leave the hospital and still be mentally charting twenty minutes later. You can replay an interaction with a patient or coworker, suddenly remember something you meant to do, or realize you have been gripping the steering wheel for half the drive. Sometimes you pull into the driveway and sit there for a few minutes before going inside.
Nurses know the driveway sit.
It is easy to look at these experiences and decide we should be better at relaxing. Instead, let’s look at it with curiosity. The environment changed but the nervous system has remained in response mode.
That makes the transition home more than just travel time. It becomes the space where the nervous system recognizes a new environment and becomes receptive to adjusting to the different context.
Predictability Can Help Mark the Change
Our nervous systems thrive on patterns because patterns make prediction easier. This does not mean we need perfectly predictable lives. It simply means repeated experiences become useful information.
The transition home becomes a place to offer cues to your body that begins to recognize over time: taking off your badge, changing your shoes, getting into the car, listening to music, driving in silence, changing clothes when you get home, walking the dog, or sitting in the driveway until you are ready to go inside.
None of these is a magic nervous system hack. We do NOT need to turn your commute into another wellness assignment. The important piece here is simply repetition. We are creating repeatable patterns that the brain will in turn recognize as a signal for new context.
This is also why forcing yourself to relax is rarely productive. If you’re sitting in traffic telling your nervous system to calm down… you know you got a big F U from your nervous system.
Instead, we want to invite observances. Curiosity gives us more useful information.
Nurses Already Know How to Do This
One of the great ironies of nursing is that we become incredibly skilled at observing other people while often becoming less aware of ourselves.
We spend our shifts noticing changes in breathing, posture, facial expression, movement, speech, vital signs, lab values, and behavior. We gather information before deciding what it means. We look for patterns rather than making conclusions from one isolated observation.
Those same assessment skills can be directed inward.
You might notice what happens during the trip home without trying to alter any of it. Perhaps your jaw softens at a certain point in the drive, or your breathing changes once you leave the hospital campus. Maybe you don’t notice you’re hungry or tired until you walk through your front door. Maybe don’t realize that your mind keeps returning to work until you change clothes, shower, walk the dog, or finally sit down.
There is no correct sequence for your body to decide the shift is over. The observation itself is useful because it tells you something about how your nervous system recognizes transitions.
That is very different from asking, “Why can’t I relax?”
The question I find more helpful is, “What am I noticing?”
Going Home Is a Physiological Transition, Too
Healthcare asks nervous systems to move through an enormous range of experiences. A nurse can go from managing an emergency to reassuring a frightened family member, from grief to humor, from intense concentration to an interruption about something completely unrelated, sometimes within the same five minutes.
We become accustomed to asking our bodies to make those transitions because the work requires it. Then we leave and expect one last transition. From nurse to whatever comes next - to happen automatically.
Sometimes it does. Sometimes the body needs more data.
There is nothing particularly dramatic about noticing when your shoulders drop during the drive home or realizing that changing out of scrubs creates relaxation in your body. That is exactly why these observations matter. They’re ordinary pieces of physiology happening in real life.
Your body is constantly communicating through tension, breathing, attention, movement, fatigue, and countless other signals. The goal is not to attempt to fix every one of them.
The goal is to become more in tune with noticing what they are telling you.
After your next shift, simply pay attention to the space between leaving work and feeling like you are actually home. It’s all part of the practice and part of your journey.
